 Before making an Appointment, please ask your Claims Manager for a copy of the letter that discusses the need for an impairment rating. 

You may also want to make sure you let your claim manager knows you will be evaluated by Dr. Slutsky for the Rating Exam

 If you are being referred by your treating provider, please make sure their office completes the LNI Treating Physician Referral Form (found below) along with the most recent medical records 

 Claim Managers and Attorneys may refer clients directly to Dr. Slutsky's office using the "Claim Manager Referral" Form or "Attorney Referral" Form (found below).
